MEDICAL RELEASE

BASELINE CONCUSSION TESTING

In addition to the medical forms above, 7th and 8th Grade Players are required to have a Baseline Concussion Test performed.  **NOTE: If your player did a BCT last season and has not had any concussion issues, last year's test is still valid**

This test can be performed at home by visiting.  https://baselinetesting.com/

Choose RESOURCES -- BASELINE TESTING.  The baseline test costs $20; proof of testing will be emailed to you and you can forward it to our registrar.

Testing is optional for 5th and 6th graders, but recommended.  Flag and Rookie Tackle players do not need a baseline concussion test.

If you prefer to have your child tested in person, please reach out to your medical care provider.
