Registration and payments are strictly online. If this is your first time registering, go to the Member Login (see tab on left) and select “NO, I do not have a member account”.    If you have previously registered with us, select "Yes, I already have a member account".  Forgot your login info?  Select "I forgot my account information".  **Please make sure to check that all of your information is up-to-date**

  
Registration forms are:

1. Registration Receipt/Player Contract This will come as an attachment after your register online. If you do not receive the confirmation e-mail with the attachment, please check your spam/bulk folder first.  You can also reprint your forms from your member account under "past registrations."  If you still don’t find it, contact Meredith Merk at registrar@lincolnyouthfootball.info. 
    Bring to equipment handout signed (please leave photo area blank).
2. Medical release form click here if you need another copy of this form. This form must be signed by a Doctor/Nurse Practitioner and returned at equipment handout.

Registration is not complete until all forms and full payment are in.  Forms must be handed in at one of the two Equipment Handout dates. 
Saturday, June 12th, West Sylvan Middle School 10-1pm
Sunday August 8th, West Sylvan Middle School 2-5pm
 

YOU WILL NOT BE ALLOWED TO PRACTICE UNTIL  BOTH YOUR SIGNED PLAYER REGISTRATION FORM AND SIGNED MEDICAL RELEASE FORM ARE HANDED IN TO THE REGISTRAR.

Refunds:
There are no refunds after August 1st. For those requesting a refund prior to August 1^st, there will be a $50 fee deducted from the registration fee paid.
